Observation

During the course of the complaint assessment it was reported that both of the teachers present at the time that the child sustained an injury to the mouth did not observe the incident that occurred on 06/06/2022. One of the teachers stated that the child approached them to report the injury and it was reported that neither teacher was aware that a child had plastic fork used during lunch. It was also reported that the parent did not receive the injury report until two days after the incident. A teacher stated that she communicated the story that the children told her to the parent the day of the injury. This created discrepancies in what the teacher verbally reported to the parent and what was reported on the injury report after the video footage was viewed. Dispute Description: We are disputing a lapse of supervision occurred based on the following facts which video evidence verifies. Both teachers were present in the classroom at the time that the child sustained an injury to the mouth, but did not observe the incident because they were both actively assisting other children in the classroom. The teachers are positioned to see all the children, and do scan the room in between helping other children. In the matter of seconds that the incident happened, one teacher was assisting a child with their face mask and the other teacher was assisting a couple of children who were getting their nap mats out and their sheets on their mats. The teacher reported the incident to the parent verbally that day upon pick up (parent happened to pick up minutes after the incident occurred). The report of what the injured child stated to the teacher was was written the next day and left for parents to pick up. We followed up with the parent after reviewing the video footage of the incident and information was available.
